Mission Neighborhood Health Center (“MNHC”) values your support and is committed to protecting your privacy. Information we collect automatically. We may collect certain information automatically when you subscribe to and access our newsletter, and other device identifiers that are automatically assigned to your computer or device when you access the Internet, the date and time of your visit, information about the links you click and pages you view within the Services, and other standard server log information.
Information submitted through patient portals or other third-party healthcare platforms linked from the Services is not collected through this website and is not subject to this Privacy Policy.
Information about how MNHC uses and discloses protected health information is described in our Notice of Privacy Practices, which is available upon request or through our clinics.
MNHC provides access to a patient portal (“MyChart”) for the convenience of our patients. The patient portal is operated and hosted by a third-party service provider and is not part of the Services covered by this Privacy Policy.
Any information you submit through the patient portal, including medical information and protected health information (“PHI”), is not governed by this Privacy Policy. Such information is governed by MNHC’s Notice of Privacy Practices and the privacy policies and terms of use of the patient portal provider.
MNHC does not control the privacy or security practices of the patient portal provider. We encourage you to review the patient portal provider’s privacy policy before submitting any information through the portal.
We only use payment information, such as credit card numbers, expiration date, and security codes, as necessary to complete a particular transaction or as otherwise required by law.

If you provide your mobile phone number to MNHC and consent to receive text messages, we may use your mobile number to send healthcare-related communications such as appointment reminders, billing notifications, care coordination messages, wellness reminders, or other information related to services we provide.

Your mobile number and text messaging consent information will not be sold or shared with third parties or affiliates for marketing or promotional purposes. Text messages sent through standard SMS or RCS channels may not always be encrypted and could present some privacy risk. For this reason, MNHC limits the content of text messages and does not send highly sensitive protected health information through standard text messaging channels unless otherwise permitted by law.

The Services are designed for and targeted to U.S. audiences and are governed by and operated in accordance with the laws of the U.S. We make no representation that the Services are operated in accordance with the laws or regulations of any other nations. By using the Services and providing us with information, you understand and agree that your information may be transferred to and stored on servers located outside your resident jurisdiction and, to the extent you are a resident of a country other than the United States, that you consent to the transfer of such data to the United States for processing by us in accordance with this Privacy Policy.
MNHC does not track its customers over time and across third party websites to provide targeted advertising. Some web browsers may transmit “do-not-track” signals to the websites with which the user communicates. However, web browsers respond to this functionality in different ways, and it is not always clear whether users intend for these signals to be transmitted. In addition, there currently is disagreement, including within the leading Internet standards-setting organization, concerning what, if anything, websites should do when they receive such signals.
